The MAT Program Coordinator will support and coordinate for the MAT services offered at LCH, including primary care behavioral health, outpatient therapy, group psychotherapy, and psychiatry. Primary responsibilities will include resource and care coordination, population health management, and administrative support for the behavioral health team, as well as participation in team meetings.
The MAT Coordinator will collaborate with members of the larger multidisciplinary LCH team as needed to advocate for each LCH member’s needs to be addressed.
This position will require travel between LCH locations.
Qualifications and Skills
- Bilingual proficiency in Spanish and English preferred
- Bachelor of Social Work, Psychology, or Public Health required
- Excellent organizational, written, and verbal communication skills required
- Computer proficiency; experience with Microsoft Office preferred
- Current Driver's License (PA, DE, or MD)
